    Let’s get something clarified here and kill the confusion. There are a LOT of branded names out there for the same thing: Full-Arch Fixed (screwed in) implant restorations that are held off the tissue and are entirely bone supported. In essence, a bridge built on implants instead of teeth.

    This in our opinion is the BEST solution for tooth replacement for many reasons including:

  • They last the longest with the least amount of long term bone loss.
  • They are the most comfortable and closest option to your natural teeth
  • They are the LEAST maintenance and do not have to be replaced near as often. The 10 year survival rate on these restorations is above 95% which means at 10 years in 95 + percentage of these are still rocking and rolling! The oldest implant bridge I have seen was placed in 1965 and still functioning!
  • They are NOT removable by you but can be removed by your doctor for maintenance and cleaning.
  • They do not absorb bacteria, fungus and other nasty stuff that dentures do.
  • They do not trap food like an implant retained denture does** (IF they are constructed hygienically with good materials. Unfortunately I see a LOT of them that are not and have to be remade!

    Let’s break down some of the options you may have heard about.

    All on 4 Dental Implants

    Revolutionizing implantology since 1998: All-on-X with added resilience. Choose expertise, not compromise.

    Dr Paulo Malo developed the technique of using tilted implants under the sinus and over the mandibular nerve exits in 1998 and coined the term “All-on-4” in collaboration with the implant company Nobel Biocare. This revolutionary and controversial idea allowed patients to receive immediate load implants without sinus grafting and get teeth in a day when it was not possible before. We use the “All-on-X” technique of tilted implants but improve upon it by adding two implants for the same price so if you do lose an implant in the future you do not end up having to start all over again.

    The saying in the implant world is “All on 4, none on 3”. In addition, we are the only restorative providers in Alaska utilizing advanced techniques such as Zygomatic and Pterygoid implants for low bone cases so that if we do not get good stability with a traditional “All-on-X” protocol, you still leave with teeth and not a denture. We see Many cases where there is insufficient bone for a standard All on 4. 

    By charging a flat fee for the final restoration regardless of number of implants, we don’t put ourselves in a box and can engineer the final restoration to last as long as possible. We sometimes place as many as 8 implants in an arch when the bone is soft. Don’t put your restoration at risk so a doctor can save a few hundred dollars on extra implant fixtures!

    3 on 6™

    Repackaged 1950s technique: 3 bridges, more risk. Rigid fixation vital for bone. Don’t sacrifice quality for outdated ideas.

    Ok this one is just a repackage and sticker on a technique that has been around since the 1950s. Interrupted, fixed implant bridges. The idea is by separating the Full Arch into 3 bridges, you minimize your risk of losing the prosthesis and only have to replace one of the bridges if you lose an implant. The problem with this reasoning is the whole structure loses the benefit of tying more implants together for rigidity.

    Rigid fixation is the best way to prevent bone loss. In addition, if you are placing 6 plus implants we can typically remove the failed implant, fill the hole and put the same bridge back in so that reasoning is flawed anyway because it typically costs you MORE since we would need to remake one of the 3 on 6 implant bridges entirely if one fails. They also market this as a way to preserve bone by not cutting down the ridge. That is also called an FP1 prosthesis and has been around a LOT longer than the people who market this technique and sell it as their idea to unsuspecting dentists.

    Yes we can often do an FP1 but that doesn’t mean its the best solution for you! There are reasons we do both. When we can we always minimize the reduction of bone. But there are times when not doing proper bone reduction results in early failure or a terrible looking smile.

    Mini Implants

    Repackaged 1950s technique: 3 bridges, more risk. Rigid fixation vital for bone. Don’t sacrifice quality for outdated ideas.

    If a provider tells you they can make you a long-term restoration on mini implants and save you money, RUN! Small diameter implants are utilized in very narrow areas of the jawbone where there is insufficient space such as the lower anterior teeth to successfully restore single tooth restorations. These areas are also protected from heavy loads by the natural teeth surrounding them. When used for snap in dentures or even full arch restorations, they are considered to be “temporary implants” and should not be expected to last beyond a few years. Unfortunately, there are many inexperienced providers who take a weekend course on mini implants and are told they are a viable long term solution for full arch rehabilitation.

    Remember, the implant screw is a precision engineered structure and the correct size and shape must be selected for the site chosen in order not cause early bone loss due to overload. We have in the past utilized small diameter “mini” implants for temporary anchorage under a denture while large diameter implants heal around them, however we now utilize advanced Anchorage and grafting techniques that make these dinky fixtures pretty much obsolete. There are better ways to save money on implants that will not result in heartache 3 to 5 years down the road. If cost is the major consideration in your care, let us know and lets talk!

    G4 by Golpa

    Golpa’s coined term ‘All-on-4’ repackage.

    The Golpa (G4)-Implant Solution consists in establishing a fully customized bridge based on, as few as, four titanium implants. The solution takes advantage of existing bone, even if scarce, avoiding having to use bone grafts. A term coined by Dr. Golpa which is just a repackaging of All on 4 with his name on it.
